Each of these forms includes questions relevant to the therapy covered.
For instance, the reflexology form asks if the client has veruccas or athlete's foot, whilst the aromatherapy form has space on the reverse side to list the oils selected and percentage used for the blend. Once completed, the client and therapist then sign the declaration section of the form before continuing with treatment.
